Discharge planning is a complex process. Hospitals must have an effective discharge planning process, as a Condition of Participation, that focuses on the patient’s goals and treatment preferences, involving the patient and their caregiver/support person(s) as active partners in the discharge planning for post-discharge care.
A successful discharge plan is crucial to reducing the risk of readmission, improving the quality and safety of hospital care, and reducing costs. This course will assist in identifying the key elements necessary to meet the Discharge Planning CoP.
