Medicare encourages hospitals to improve transitions and care coordination to better engage patients and caregivers in discharge plans and, in turn, reduce avoidable readmissions.
This course will help attendees learn how to better manage hospital readmission. Join Mayra Pineiro to discuss readmission risk factors, readmission reduction strategies, and review examples of interventions that have successfully reduced readmission rates for patients who were discharged to their home.
